What is Support Coordination?
Support Coordination helps NDIS participants understand and implement their plans, connect with providers, and work towards their goals.
Understanding Support Coordination
Support Coordination is a funded support in your NDIS plan that helps you understand and use your NDIS funding to pursue your goals.
A Support Coordinator works with you to:
- Understand what's in your NDIS plan and how your funding works
- Find and connect with service providers that match your needs
- Build your skills in managing your own supports
- Prepare for your next NDIS plan review
- Resolve any issues with your services or providers
- Link you with community and mainstream services
Types of Support Coordination
Support Connection
For participants who: Need some help to get started with connecting to providers and understanding their plan.
Support Connection is a short-term support (usually 3-6 months) that helps you get your plan up and running. Your Support Connector will help you understand your plan, find providers, and get services started.
Once you're connected and comfortable, this support ends and you manage your own plan.
Coordination of Supports
For participants who: Need ongoing help to coordinate multiple providers and services.
This is the most common type of Support Coordination. Your Support Coordinator works with you throughout your plan to help you:
- Connect with and manage multiple service providers
- Monitor your funding and service agreements
- Problem-solve when issues arise
- Prepare for plan reviews and reassessments
- Build your capacity to eventually manage your own supports
Specialist Support Coordination
For participants who: Have complex support needs or circumstances requiring specialist expertise.
Specialist Support Coordination (SSC) is for participants with very complex needs, such as:
- Multiple service providers across different sectors
- Complex behaviours or mental health needs
- Challenging life circumstances
- High-risk situations requiring urgent intervention
- Forensic or justice-related needs
SSC Coordinators have advanced skills and can provide more intensive support.
Not Sure Which You Need?
The type of Support Coordination you receive is decided by the NDIS based on your needs. If you're not sure what's in your plan or what type you have, we can help you understand your funding.
How We Work with You
Our approach is person-centred, strengths-based, and focused on building your capacity and independence.
Understanding Your Goals and Plan
We start by getting to know you, your goals, and what you want to achieve with your NDIS plan. We'll help you understand your funding categories and how to use them.
Exploring Service Options
We help you explore different service options and providers that align with your goals. We can provide information about providers' services, locations, and availability.
Connecting with Providers and Supports
We support you to connect with providers, help you understand service agreements, and ensure services are working well for you. We can attend initial meetings with providers if you'd like.
Ongoing Support and Monitoring
We check in regularly to see how your services are going, help resolve any issues, monitor your funding, and adjust your supports as your needs change.
Plan Reviews and Preparation
As your plan review approaches, we help you gather evidence, prepare your story, and communicate your goals and needs clearly to the NDIS.
Building Your Capacity
Throughout our work together, we focus on building your skills and confidence so you can increasingly manage your own supports. Our goal is to support you towards independence.
How is Support Coordination Funded?
Support Coordination is funded under Capacity Building - Support Coordination in your NDIS plan. This funding is separate from your Core Supports and can only be used for Support Coordination services.
The amount of Support Coordination funding you receive depends on your individual needs and circumstances. It's determined by the NDIS when they create or review your plan.
Important: Support Coordination funding cannot be used for other supports, and other supports cannot be used to pay for Support Coordination.
