About NDIS Support Coordination
Yes. Support Coordination is a specific funded support in your NDIS plan under "Capacity Building - Support Coordination". If you're not sure whether you have this funding, check your plan or contact the NDIS. We can also help you understand your plan if you'd like to get in touch.
If you don't currently have Support Coordination funding but think you need it, you can request it at your next plan review.
Support Connection is short-term support (3-6 months) to help you get started with your plan and connect with providers.
Coordination of Supports is ongoing support throughout your plan to help you coordinate multiple services, monitor your funding, and prepare for plan reviews.
Specialist Support Coordination is for participants with very complex needs who require intensive support and specialist expertise.
The type you receive is determined by the NDIS based on your needs. Learn more about the types.
Support Coordination helps you understand your plan, find providers, and work towards your goals.
Plan Management is a financial service that pays your providers and manages your invoices and budgets. Plan Managers don't help you find services or work on your goals.
You can have both Support Coordination and Plan Management if both are funded in your plan. They work together but provide different services.
Working with Afterlight Support
We primarily serve Melbourne and surrounding regions. However, we can work with participants in regional Victoria if we can provide services via phone or video call, or if you're willing to travel to meet us.
Contact us to check if we can support you in your area.
Yes! We offer flexible service delivery including phone calls, video calls (via Zoom or similar), and in-person meetings. We can use a mix of methods depending on what works best for you.
Many participants prefer video calls as they're convenient and still allow face-to-face connection. Others prefer in-person meetings. We'll work with your preference.
Absolutely. We welcome family members, carers, and support people to be involved if that's what you want. Many participants like to have someone they trust with them in meetings, especially when making important decisions.
You're always in control of who's involved and how much information is shared with others.
It depends on your needs and preferences. Some participants like fortnightly check-ins, others prefer monthly meetings, and some only need occasional contact.
When we first start working together, we'll meet more frequently to get to know you and get your plan set up. As you become more confident, we might meet less often.
You can always contact us between meetings if something comes up or you need help.
You have the right to choose your Support Coordinator and change providers at any time. We won't be offended if we're not the right fit for you.
If you're thinking about changing, we'd appreciate the opportunity to discuss your concerns first. Sometimes issues can be resolved. But if you do decide to change, we'll help make the transition smooth and share relevant information with your new coordinator (with your consent).
If English isn't your first language or you'd prefer to communicate in another language, we can arrange a professional interpreter at no cost to you. We want to make sure you fully understand everything and can communicate clearly.
Just let us know what language you need when you contact us, and we'll arrange an interpreter for our meetings.
NDIS Plans and Funding
No. Support Coordination is funded separately under "Capacity Building" in your plan. It doesn't take away from your Core Supports funding or other Capacity Building categories.
Your Support Coordination funding can only be used for Support Coordination—it can't be used for other supports like personal care or therapy.
We'll help you prepare for your plan review well before your current plan ends. We'll work with you to:
- Gather evidence of progress towards your goals
- Document what's working and what's not
- Identify any new or changing needs
- Prepare your story for the NDIS
If you want Support Coordination to continue in your new plan, we'll help you request it in your planning meeting.
We monitor your Support Coordination funding throughout your plan and will let you know if we're getting close to the limit. If we think you'll run out before your plan ends, we can:
- Adjust the frequency of our meetings
- Focus on priority issues
- Help you request a plan review if your circumstances have changed
We aim to pace our support so your funding lasts for your entire plan period.
Getting Started
It's easy to get started:
- Contact us via phone, email, or our contact form
- We'll arrange an initial phone call or meeting to learn about your needs and explain our services
- If you'd like to work with us, we'll send you a Service Agreement to review and sign
- We'll schedule your first official meeting to start working on your goals
The whole process usually takes less than a week from first contact to your first meeting.
For your first meeting, it's helpful to have:
- Your NDIS plan (or NDIS number so we can access it online)
- Any service agreements with current providers
- Information about your goals
But don't worry if you don't have everything—we can help you track down any information you need.
No. Initial conversations to discuss your needs and our services are free. There's no obligation, and we won't charge you anything unless you decide to engage us as your Support Coordinator.
