Support Connection

Short-term support to get your plan started

Who is this for?

Support Connection is ideal for participants who:

  • Are new to the NDIS or have a new plan
  • Have straightforward support needs
  • Need help getting started but feel confident managing ongoing
  • Want to build skills quickly to self-manage

What you can expect

  • Help understanding your NDIS plan and funding
  • Support to find and connect with service providers
  • Assistance setting up service agreements
  • Information about local supports and services
  • Skills building to manage your own supports

This support typically lasts 3-6 months, after which you'll be confident managing your own plan.

Working with families and carers

We welcome family members, carers, and support people to be involved. We'll work together to ensure everyone understands the plan and how to access supports.

Coordination of Supports

Ongoing support to coordinate multiple services

Who is this for?

Coordination of Supports is for participants who:

  • Have multiple service providers
  • Need help coordinating different supports
  • Want ongoing support throughout their plan
  • Face challenges managing their services alone
  • Need help problem-solving service issues

What you can expect

  • Regular check-ins (frequency based on your needs)
  • Help finding and connecting with providers
  • Support to resolve issues with services
  • Monitoring of your funding and service delivery
  • Coordination between different providers
  • Preparation for NDIS plan reviews
  • Links to community and mainstream services
  • Crisis support when urgent needs arise

Working with your supports

We collaborate with your family, carers, other service providers, and support networks. We can attend provider meetings, participate in care team discussions, and ensure everyone is working together towards your goals.

Specialist Support Coordination

Intensive support for complex needs

Who is this for?

Specialist Support Coordination (SSC) is for participants with very complex needs, including:

  • Multiple providers across health, disability, and other sectors
  • Complex behaviours requiring specialist behaviour support
  • Psychosocial disability or complex mental health needs
  • High-risk situations or safeguarding concerns
  • Justice or forensic involvement
  • Unstable housing or homelessness
  • Multiple hospitalisations or crisis presentations

What you can expect

  • More frequent and intensive support
  • Specialist expertise in complex needs
  • Strong advocacy and system navigation
  • Crisis intervention and support
  • Coordination across multiple service systems
  • Support for families and carers
  • Transition support (e.g., hospital to community)
  • Links to specialist providers and supports

Our specialist team

Our Specialist Support Coordinators have advanced qualifications and experience in complex disability support, mental health, behaviour support, and working with marginalised communities.

Flexible Service Delivery

We understand that everyone has different preferences and circumstances. We can meet with you:

  • At your home
  • At our office
  • In the community (café, library, etc.)
  • Via phone or video call
  • At a time that suits you (including evenings and weekends where possible)

Language and Communication

We're committed to accessible communication. We can:

  • Arrange interpreters for non-English speakers
  • Provide information in Easy Read format
  • Use communication boards or assistive technology
  • Allow extra time for appointments
  • Welcome support people in meetings
